Choking, medically termed foreign body airway obstruction (FBAO), happens when something blocks the respiratory tract and prevents air from reaching the lungs. While the body’s stored oxygen can sustain life for a few minutes after breathing stops, choking frequently results in death. It ranks as the fourth most common cause of unintentional injury death in the United States.
Each year, roughly 4,500 to 5,000 people in the U.S. die from choking. The highest death rates occur in children under three and adults over 75. Foods that can mold to the shape of the throat—like bananas, marshmallows, or gelatinous candies—are especially hazardous. Specific first-aid techniques exist to treat choking. Many episodes are brief and resolve without medical help, so they go unreported. Among reported cases, 80% involve people under 15, and 20% involve those older. Worldwide in 2013, choking on a foreign object caused about 162,000 deaths (2.5 per 100,000 people), up from roughly 140,000 deaths (2.9 per 100,000) in 1990.
**Signs and symptoms** Choking can be subtle, especially when a foreign body has been lodged for a long time. Coughing occurs in 80% of cases, and shortness of breath in 25%. Victims may be unable to speak, use hand signals to indicate choking, try to vomit, or clutch at their throat.
**History of the episode** In about 90% of choking events, someone witnesses or recalls a sudden onset of respiratory and skin symptoms while eating or handling small objects. Initial episodes last seconds to several minutes, but symptoms can improve temporarily, which may be mistaken for full recovery.
**Respiratory signs** Early respiratory signs include involuntary cough, gurgling, gagging, shortness of breath, labored breathing, or wheezing. Children often drool excessively or have stridor (high-pitched breathing sounds). The classic triad in children—coughing, wheezing, and diminished breath sounds—appears together in only about 60% of cases, according to a 10-year review. If breathing is not restored, the victim may lose consciousness. With lower airway aspiration, pneumonia-like symptoms can develop, such as fever, chest pain, foul-smelling sputum, or blood in sputum (hemoptysis).
